Study Links Whole Grains To Multiple Heart Benefits

The systematic review and meta-analysis included 87 randomized clinical trials involving 6,529 participants across Asia, Europe, the United States, Canada, Australia and Brazil. Researchers examined how whole-grain intake affected cardiovascular risk factors, providing new insights into the potential benefits of Whole Grains for Heart Health.

People who consumed more whole grains generally showed lower body weight, smaller waist circumference and lower blood pressure. The analysis also found reductions in total cholesterol, LDL cholesterol, triglycerides, blood glucose and interleukin-6, a marker of inflammation.

The study found measurable benefits at 30 to 40 grams of whole grains per day. The strongest effects generally appeared between 60 and 100 grams, equivalent to about four to six servings.

That amount could come from foods such as oatmeal, whole-grain bread and brown rice, according to the researchers.

Researchers Identify A Practical Daily Range

Dr. Helda Tutunchi of Tabriz University of Medical Sciences, who led the research, said the analysis addressed a question left unclear by earlier studies: how much whole grain people need to consume for meaningful cardiovascular benefits. “This is the largest and most comprehensive meta-analysis of randomized controlled trials on wholegrain intake and cardiovascular risk factors,” Tutunchi said. She added that the greatest benefits appeared at about 60 to 100 grams per day.

The researchers found high-certainty evidence for improvements in body weight, waist circumference and total cholesterol. They found moderate-certainty evidence for changes in LDL cholesterol, triglycerides, systolic blood pressure, fasting blood glucose and insulin resistance.

Tutunchi said the findings do not necessarily require major changes to existing dietary guidelines. Instead, they support efforts to help people increase whole-grain consumption toward the higher end of current targets, highlighting the potential role of Whole Grains for Heart Health.

The analysis also found that whole grains may work through several modest changes rather than one major effect on a single heart-risk factor.

Short Trials Limit Long-Term Conclusions

The researchers cautioned that most trials lasted for relatively short periods, with an average duration of about eight weeks. The studies measured cardiovascular risk factors rather than actual heart attacks, strokes or other cardiovascular events.

As a result, the findings do not establish whether higher whole-grain intake will directly reduce cardiovascular disease events over many years. More long-term research is needed to test whether the observed changes persist.

Dr. Cecilie Kyrø of the Danish Cancer Institute in Copenhagen and colleagues made a similar point in an accompanying editorial. They said replacing refined grain foods with minimally processed whole grains is likely to produce modest but meaningful improvements in body weight, waist size and blood lipids.

Kyrø and her colleagues also noted that whole grains retain the bran, germ and endosperm found in intact cereal grains. They cautioned that foods marketed as containing whole grains can still be highly processed.

The researchers said the findings support a practical dietary shift toward minimally processed whole-grain foods while more evidence develops on long-term cardiovascular outcomes.
