Men genetically predisposed to male-pattern hair loss may face a 7% higher risk when GLP-1 activity is increased, according to research published Thursday.

Genetic analysis finds higher hair-loss risk

Researchers led by Lynn Petukhova of NYU Grossman School of Medicine analyzed genetic data from more than 230,000 people to examine whether increased activity of the GLP1R gene, which encodes the receptor targeted by GLP-1 drugs, is linked to androgenetic alopecia, the most common form of male-pattern hair loss.

The study, published in the Journal of Investigative Dermatology, used two large public genetic databases rather than tracking men who were directly taking GLP-1 medications. Researchers used inherited genetic variants associated with higher GLP1R expression as a proxy for increased GLP-1 activity.

The researchers found that men with greater GLP1R activity had a 7% higher likelihood of androgenetic alopecia. The association remained after researchers accounted for other potential contributors, including hypertension, insulin resistance and lower testosterone levels.

“Our study provides the first genetic link between GLP-1 use and an increased risk of male-pattern hair loss,” Petukhova said. She cautioned that additional research is needed to establish how GLP-1 activity could affect hair follicles.

Earlier research also flags hair thinning

The findings add to a growing body of research examining GLP-1 hair loss among people using drugs such as Ozempic, Wegovy and Zepbound. A separate systematic review published in Science Progress in April analyzed 24 studies and found that semaglutide and tirzepatide had the strongest hair-loss signals among GLP-1 drugs.

That review identified telogen effluvium, a form of temporary hair shedding, and androgenetic alopecia as the most frequently reported types. It also found that rapid weight loss may contribute to hair shedding, particularly with tirzepatide.

Dermatologist Paradi Mirmirani, one of the review’s authors, said GLP-1 hair loss associated with the therapy may often result from rapid weight loss rather than a direct drug effect. She said most cases of telogen effluvium are temporary and can improve after weight stabilizes.

A July meta-analysis provides additional evidence of an association. It analyzed nine interventional studies involving 4,114 GLP-1 users and found a higher risk of GLP-1 hair loss among users than placebo groups, although the researchers said more research is needed to understand the relationship.

Researchers urge more study before drawing conclusions

The new genetic study does not show that every man taking a GLP-1 drug will lose hair. Instead, it points to a possible genetic susceptibility and provides evidence for a biological connection that researchers say needs confirmation through further studies.

Petukhova said future research could determine whether some patients should have their hair-loss risk assessed before receiving GLP-1 medications. She also said researchers may eventually explore preventive treatments for people considered particularly vulnerable.

The findings come as GLP-1 medications continue to gain widespread use for obesity and Type 2 diabetes. Researchers say understanding potential GLP-1 hair loss risks will become increasingly important as more patients use the drugs for long-term weight management.
